Inschrijven praktijk

Onze praktijk is geopend voor nieuwe patiënten die recent naar Leimuiden zijn verhuisd en wonen in het postcodegebied 2451 en 2465.  Partners en kinderen van mensen die reeds patiënt bij ons zijn, kunnen natuurlijk ook worden ingeschreven.
Wij moedigen het gebruik van het patienten portaal of de Uw Zorg Online-app.  sterk aan, omdat het voor u en voor  ons efficiënt werkt en tijd scheelt. Via het portaal of Uw Zorg Online-app kunt u uw dossier inzien, een afspraak maken, een vraag stellen en herhaalmedicijnen bestellen.

Wilt u zich inschrijven als patiënt van onze praktijk? Vul dan dit formulier in.
Als u zich inschrijft als nieuwe patiënt(e) kunt u een afspraak maken voor een kennismakingsgesprek. Uw medische voorgeschiedenis en medicatiegebruik en uw verwachtingen kunnen dan besproken worden.

Indien u verhuist en uit Leimuiden of Rijnsaterwoude vertrekt, neem dan contact op met de praktijk. Wij kunnen dan uw gegevens naar de nieuwe huisarts sturen.

Our practice is open to new patients who have recently moved to Leimuiden and live in the postcode areas 2451 and 2465. Partners and children of people who are already patients with us are of course also welcome to register. If you are moving away from Leimuiden or Rijnsaterwoude, please contact the practice. We can then send your records to your new GP.
